Job summary

hammondcare in Hervey Bay offers a Dementia Support Coach role on a fixed-term contract through June 2028, with hybrid work arrangements and a focus on transitioning people with dementia from hospital settings to home or residential care.

You will lead on-the-ground support, coaching, and relationship-based care, collaborating with staff and carers to implement best practices and ensure smooth delivery of dementia services across settings.

Qualifications

• Qualification in a relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care or similar highly desirable.
• Current driver’s licence and safe driving record required.
• Willingness to travel intrastate and interstate to support DC services.

Responsibilities

• Facilitate on-the-ground support and modelling of relationship-based strategies with clients, carers and care staff.
• Collaborate with peers to develop skills and applied knowledge of aged care staff and carers.
• Support implementation of recommendation plans and seek guidance from senior staff as required.
• Facilitate and support brokerage items with clients and carers, under supervision.

Responsibilities

• The Dementia Support Coach will facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff.
• Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
• Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
• Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.

What do we need from you?

• Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
• The ability to work both independently and as part of a team.
• Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
• A current driver's licence and safe driving record
• Willingness to travel and work intrastate and interstate to support the delivery of DC services
• Qualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable
