HammondCare is seeking a Dementia specialist in Woy Woy for a permanent full-time role. You will join our on-site team, providing dementia-specific advisory support and coaching to staff, while contributing to regular clinical reviews and quality improvements.

The role emphasises engagement planning, teamwork, and participation in projects and behavioural management planning within our residential setting. RN or Allied Health qualifications are essential.

• 3 years of proven dementia care experience is essential.
• Registered Nurse or Allied Health qualification is essential; Training & Assessment is desirable.
• Strong teaching, mentoring and coaching experience is required.
• Skills in resident care tasks and effective communication are expected.
• Excellent interpersonal, negotiation and relationship-building skills.
• Proficiency with Microsoft Office tools.

Responsabilidades

• Provide dementia expert advisory services on the ground through clinical reviews.
• Coach staff to build capacity for dementia care and escalation management.
• Plan engagement with individuals and teams to meet practice objectives.
• Participate in Residential team meetings, supervision and activities.
• Evaluate and report using HammondCare processes for delegation and projects.
• Participate in projects and behavioural management planning.

About the role
A rewarding opportunity for a passionate Dementia professional to join HammondCare on anPermanent Full-timebasis at our Woy Woy location.

Some other responsibilities include:

• On the ground support. Providing dementia specific expert advisory services within HammondCare services, through participation in regular clinical review meetings
• Coaching staff, building their ability and capacity to deal with dementia related situations and escalations. Build staff capacity through communication, education, mentoring and coaching in practice; this can include spending time with the team on the ground and lead by example with care support, but also conducting case study discussions and review of documentations
• Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements
• Participate, contribute and learn as part of the Residential team in team meetings, professional supervision and team activities
• Evaluation and report by utilising HammondCare processes to communicate, monitor and manage work delegations and projects including evaluation and reporting
• Participating in projects and behaviour management planning

What do we need from you?

• 3 years of Proven experience working with people living with Dementia is essential
• Registered Nurse or Allied Health qualification essential. Further qualification in Training and Assessment is highly desirable
• Demonstrated skills in teaching, mentoring and coaching others
• Confidence with conducting care tasks such as personal care and resident general support
• High level of negotiation, communication and relationship building skills
• Problem solving and decision-making skills relevant to the aged and dementia care context
• Strong interpersonal and communications skills both written and verbal
• Strong administrative and computer skills using Microsoft Office products
• Ability to work independently and as part of a team
