This role involves improving continuity of care for patients with drug and alcohol problems exiting correctional facilities. The Connections Program aims to improve continuity of care for patients who are in public correctional Centre with Drug and Alcohol problems, that are being released to the community. The Connections Program works within an assertive linkagemodel that aims to link patients with relevant health and welfare service providers pre and post release.
The Connections Transition Coordinator will utilise a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs.
Your salary of $112,396 (Year 1) - $116,038 (Year 2) for Social Worker, Counsellor and Occupational Therapist classifications or $77,358 (Year 1) to $113,226 (Year 9) for Psychologists + super and leave loading is just the beginning. You’ll also have access to:
