Job summary

Uniting Vic.Tas in Cranbourne, VIC seeks a Complex Needs Specialist Practitioner to provide holistic case management for families of children with complex disability needs. You will assess needs, develop Family Action Plans and support navigation of the NDIS while advocating for appropriate supports.

Collaborating with multidisciplinary teams, you will implement behaviour and parenting strategies, coordinate care, and ensure compliant service delivery while engaging stakeholders and promoting

Qualifications

  • Degree in Disability, Social Work, Psychology or equivalent.
  • Current driver’s licence valid in Victoria.
  • Experience as a practitioner and case manager in disability, child, youth and family welfare.
  • Experience engaging with families and providing outreach services.
  • Experience implementing positive behaviour support.

Responsibilities

  • Provide holistic case management to families of children with complex disability needs.

Your new role

As a Complex Needs Specialist Practitioner, you will provide holistic case management to families of children with complex disability needs, supporting safety, wellbeing, and stability. You will undertake therapeutic assessments, develop individualised Family Action Plans, and assist families to navigate the NDIS while advocating for appropriate supports.

Working collaboratively with multidisciplinary professionals, you will implement behaviour and parenting strategies, coordinate care teams, and broker services aligned to family needs. You will also promote the voice of children and young people, maintain accurate records, participate in supervision, and ensure high-quality, compliant service delivery through strong stakeholder engagement and adherence to organisational standards.

Your skills and experience

You will meet the person specification in the position description, including having a degree in Disability, Social Work, Psychology or equivalent.

You Will Also Have
  • Current driver’s licence valid in Victoria
  • Demonstrated ability as a practitioner and case manager in the disability, child, youth and family welfare field.
  • Demonstrated experience engaging with families and successfully providing outreach services, sometimes in a difficult or stressful context.
  • Demonstrated experience supporting children and young people with complex needs related to their disability implementing positive behaviour support
