Job summary

Chosen Family is seeking a female Support Worker to provide person-centred care, mobility support and community access for a participant in Dural, NSW. This casual, entry-level role offers weekday coverage with some Sunday hours.

You will assist with personal care, mobility transfers, appointments and transport, and community outings while maintaining dignity and independence. A current NDIS Worker Screening Check and First Aid are required.

Qualifications

• Experience providing personal care.
• Experience supporting people with mobility needs.
• Clear spoken English communication.
• Reliable availability for the agreed roster.
• Ability to work in home and community settings.

Responsibilities

• Assist with personal care and daily living routines.
• Provide mobility support and safe transfers as required.
• Help with household tasks and daily routines at home.
• Accompany to medical appointments and essential commitments.
• Support outings, errands and community participation.
• Encourage independence and informed choice for the participant.
• Build a respectful, trusting relationship and document shift notes.
