Job summary

Power of Grace Care Services seeks a warm, patient Disability Support Worker to provide individualised 1:1 support for a paediatric participant who has recently commenced Core Supports. You will build a trusting relationship with the participant and family, support communication, daily living activities, and social participation.

The role emphasizes safety, dignity, and person-centred practice, with a meet-and-greet before regular shifts to ensure smooth matching and consistent care.

Qualifications

• Experience supporting children with disability or complex needs.
• Ability to follow therapy, Behaviour Support and risk-management plans.
• Strong communication with families, clinicians and support team.
• Reliable, punctual and able to provide ongoing support.

Responsibilities

• Provide safe, person-centred 1:1 support tailored to the paediatric participant.
• Build a trusting relationship with the participant and family.
• Supervise and manage safety risks in home and community settings.
• Assist with daily living activities and personal care as required.
• Support participation in community access, play and social activities.
• Implement strategies from allied health and Behaviour Support teams.
• Maintain clear communication and timely progress notes.
• Follow participant plans and privacy requirements.
