Job summary

Community Worker – Full-Time

Total 247 Care Pty Ltd is seeking a permanent full-time Community Worker to provide community-based and in-home support services to clients in accordance with individualised care and support plans.

Key Responsibilities

• Strong communication and interpersonal skills
• Ability to provide person-centred support and work effectively with clients, families and multidisciplinary teams
