Job summary

Garden Village in Port Macquarie is seeking a Community Registered Nurse to deliver safe, person-centred nursing care in the community setting. The role covers clinical assessment, care planning, implementation and evaluation to support clients' health, independence, and quality of life.

You will conduct comprehensive assessments, develop care plans with clients and families, provide evidence-based nursing care including medication management and wound care, and liaise with GPs and allied health

Qualifications

• AHPRA Registered Nurse with current registration.
• Excellent communication and interpersonal skills.
• Understanding of Resident Rights and Aged Care Standards.
• National Police Check and NDIS Workers Check.

Responsibilities

• Conduct comprehensive nursing assessments and identify clients’ clinical, health and wellbeing needs.
• Develop, implement and regularly review individualised care plans in collaboration with clients, families, carers and the broader care team.
• Provide evidence-based clinical nursing care, including medication management, wound care, health monitoring, chronic disease management and other nursing interventions as required.
• Monitor and evaluate clients’ health status, identifying changes or deterioration and escalating concerns appropriately.
• Provide health education, support and advice to clients and their families/carers to promote health, independence and self-management.
• Liaise and collaborate effectively with GPs, allied health professionals, support workers and other relevant healthcare providers to ensure coordinated and integrated care.
• Maintain accurate, timely and comprehensive clinical documentation and client records in accordance with organisational requirements and relevant legislation.
• Ensure all nursing practice complies with organisational policies and procedures, professional standards, clinical guidelines and applicable legislation.
• Contribute to the ongoing review and improvement of clinical practices, quality and safety within community services.
• Work autonomously in the community while maintaining effective communication and collaboration with the broader multidisciplinary team.
• Promote a positive, respectful and client-centred approach in all interactions with clients, families, carers and colleagues.
• Identify and report risks, incidents, safeguarding concerns and changes in client circumstances in accordance with organisational processes.

Candidates will require:

• AHPRA registration as a Registered Nurse
• Excellent communication and interpersonal skills - with residents and families
