Healina Health is seeking a passionate Registered Nurse to join our community care team on the Sunshine Coast. You will provide in-home nursing services, assess needs, and develop care plans while working autonomously within a supportive multidisciplinary team.

We offer a genuine 3–4 day week, flexibility, and the opportunity to make a meaningful difference in older clients’ lives. Applicants must hold AHPRA registration and have current CPR/First Aid.

Qualifikationen

• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
• Experience in community care, aged care, chronic disease management, or similar autonomous environment (highly regarded)
• Ability to work independently and manage competing priorities
• National Police Certificate or ability to obtain one
• Current Australian Driver's Licence
• Current First Aid and CPR certification (or willingness to obtain)

Aufgaben

• Provide high-quality Registered Nursing services in the community, including client assessments, care planning, interventions, documentation and communication
• Coordinate nursing interventions within scope of practice (wound care, medication management, chronic disease management, continence care, health monitoring)
• Develop and review person-centred care plans with clients and support networks
• Organise and prioritise client caseloads according to service needs and clinical priorities
• Identify changes in health status and initiate appropriate interventions, referrals, and escalations
• Communicate effectively with clients, families, GPs, and the multidisciplinary team
• Provide guidance and support to Enrolled Nurses, Allied Health Assistants, and Support Workers
• Maintain accurate clinical documentation in line with standards and policies
• Participate in professional development, quality improvement, and service enhancement
• Join rotating weekend roster (approx. one weekend per month)
