Silverchain is seeking an Occupational Therapist to join our Multidisciplinary Care team. You will oversee reablement programmes, helping clients re-engage with life in the community while collaborating with therapists, nurses and support staff.

We offer a not-for-profit environment with home-based services, flexible scheduling and a vehicle provided for work-related travel. Candidate must be an allied health professional with strong communication and organisational skills.

Qualifications

• Qualified allied health professional with field experience.
• Strong communication and teaching abilities to support clients and carers.
• Ability to plan, prioritise and work autonomously within a team setting.

Responsibilities

• Create and oversee reablement programmes for clients.
• Help clients achieve personal goals to re-engage with the community.
• Coordinate care with multidisciplinary team to ensure best outcomes.
• Provide education and coaching to direct care staff and carers.
