Job summary

Barwon Health in Geelong is seeking a motivated Community Nurse to join our Transition to Specialty Practice Program. You will undertake comprehensive assessments, develop individualised care plans, and deliver patient-centred care across home-based settings in the Geelong and Surf Coast regions.

Two permanent part-time positions available across North Geelong and Belmont Community Health Centre, with a four-week rotating roster including day, afternoon, and weekend shifts.
