Job summary

Mildura Base Public Hospital invites applications for a Mental Health Clinician to join the Adult Recovery and Wellbeing Service. The role manages a caseload and delivers case management, treatment and care coordination within a multidisciplinary team.

Applicants should hold a nursing, OT, psychology or social work degree with current registration and postgraduate mental health training. The position offers permanent full-time status and a supportive, community-focused work environment.

Qualifications

• A degree in Nursing, Occupational Therapy, Psychology or Social Work, with current AHPRA registration (or eligibility for AASW membership for Social Workers)
• Relevant postgraduate training in mental health
• Demonstrated experience providing direct clinical care to people living with mental illness
• Sound knowledge and application of recovery-oriented practice
• Strong interpersonal skills, with the ability to engage consumers and liaise with families, carers, health professionals and community agencies
• The ability to work in a multidisciplinary team, including joint case work across agencies
• Well-developed written communication and computer skills, including client management systems
• Working knowledge of the Mental Health and Wellbeing Act 2022 (Vic)
• A commitment to mental health and wellbeing promotion in the community
• Experience in community case management will be highly regarded.

Responsibilities

• Complete comprehensive psychosocial, clinical and risk assessments, and develop and review Individual Recovery Plans with consumers, families, carers and the treating team
• Coordinate access to discipline-specific services, housing, AOD, primary health and community supports, and advocate for consumers as they navigate complex systems
• Support smooth transitions into community care from our acute and inpatient teams and other referral pathways
• Monitor progress against goals and outcome measures, and keep documentation accurate and up to date
• Build strong working relationships with internal teams and external agencies, and contribute to service development and quality improvement
