We are searching for a talented Community Health Worker-Behavioral Health
Hours:40hrs
Shift: Flexible shifts & hours; 8:30am - 5:00pm
Location: Psychiatry Services – Dartmouth, MA
Travel: Driving on average 3 or more times per week required

Under the direction of the Director, Integrated Behavioral Health Operations, work as a member of the primary care team to expand organizational support for equitable and accessible health management of behavioral health (BH) and social determinants of health (SDOH) for all patients in a given practice(s). Provide culturally appropriate health education and utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patients plan of care. Work in a team-based role and participate in team activities such as huddles, pre-visit planning, population health management, and process improvement.This position will support Brain & Spine's new memory care program that focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities.This position will support in-home safety assessments of assigned members.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation: Pay rate will be determined based on level of experience.
