Job summary

Hunter New England Local Health District is seeking a Community Health Registered Nurse to deliver person-centred nursing across Singleton and surrounding rural communities, in clinic and home settings. You will provide immunisations, health assessments and care coordination to support independence and wellbeing.

The role offers variety, autonomy, and collaboration with multidisciplinary teams, with a focus on evidence-based practice and professional development within a supportive workplace.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Accredited Nurse Immuniser (NSW), or willingness to complete an approved immunisation course within 12 months.
• Experience in community or primary healthcare nursing.
• Ability to work independently and manage a diverse caseload.
• Excellent communication, assessment and clinical decision-making skills.

Responsibilities

• Deliver community nursing services across clinic and home-based settings.
• Provide immunisation services in accordance with NSW Health guidelines.
• Undertake comprehensive health assessments and develop person-centred care plans.
• Deliver chronic disease management and preventative health programs.
• Perform wound assessment and management using evidence-based practices.
• Coordinate care and facilitate referrals to appropriate health and community services.
• Provide health promotion, education and self-management support to individuals and families.
• Work autonomously and within a multidisciplinary team to achieve positive health outcomes.
• Maintain accurate clinical documentation and contribute to quality improvement initiatives.
