Job summary

Communify Queensland is seeking a Community Access Worker to provide flexible, person-centred support for people living with disability or psychosocial challenges in the Brisbane region.

You will assist with daily living, help participants engage with community activities, and promote independence while maintaining accurate records and building collaborative relationships with families and service providers.

Qualifications

• Experience in disability sector.
• Ability to support daily living activities with empathy.
• Open QLD driver’s licence required and reliable vehicle.

Responsibilities

• Provide outreach and in-home support tailored to participants.
• Assist with activities such as shopping, meal preparation, bills, and personal care prompting.
• Support access to community, education, employment, and recreation opportunities.
• Encourage capacity building and personal goal achievement.
• Complete accurate shift notes and administrative records.
• Build positive relationships with participants, families, colleagues and external providers.
