Job summary

Amity Care Services on the Mornington Peninsula is seeking a Clinical Support Nurse (RN or Endorsed Enrolled Nurse) to join our clinical team in Somerville, VIC. You will conduct participant assessments, assist with care plans, and coordinate services to achieve health outcomes while mentoring Disability Support Workers.

The role emphasizes person-centred care, clinical governance, and ongoing improvement with on-call duties as needed.

Qualifications

• Experience in disability, community services, aged care, or complex health care.
• Strong clinical assessment and care planning capabilities.
• Excellent communication and relationship-building skills.

Responsibilities

• Clinical care & assessments including reviews and ongoing monitoring.
• Developing and reviewing support and care plans with appropriate risk assessments.
• Assist with medication management, wound care, PEG feeding, catheter care as required.
• Coordinate services with participants, families, and health professionals.
• Provide staff guidance, inductions, and competency assessments.
• Contribute to governance, quality improvements, and compliant documentation.
• Participate in an on-call roster when required.
