Central Adelaide Local Health Network (CALHN) is seeking a Clinical Nurse to join the Integrated Care Geriatric Services team in Adelaide. You will support complex clients through assessment, triage and coordinated care, building partnerships across health, government and community services.

You will bring current AHPRA registration, extensive nursing experience with older adults, and strong assessment skills to lead care across clinic, community and integrated settings.

Qualifications

Registered Nurse with current AHPRA practising certificate.
Post-registration experience in a relevant nursing setting.
Experience caring for older people in varied settings.

Responsibilities

Triage and assess complex health needs.
Coordinate care across services.
Deliver clinical interventions.
Support hospital avoidance and early discharge.
Build resilience through restorative care.
Partner with multidisciplinary and community teams.
Manage short term case coordination where needed.
Work across Integrated Care sites as required.
