Job summary

Princess Alexandra Hospital in Brisbane is seeking a Clinical Nurse to join the General Practice Liaison Officer Program. You will provide expert clinical advice, coordinate care across primary, community and secondary services, and lead initiatives to improve patient pathways.

You will build strong relationships with clinicians, stakeholders and General Practice, offering education and support to enhance healthcare integration and referral processes within Metro South outpatient services.

Your new role:As a Clinical Nurse within the General Practice Liaison Officer Program, you will play a key role at the interface between primary, community and secondary care. Working autonomously, you will provide expert clinical advice, coordination and problem-solving to support General Practice and Metro South outpatient services. You will build strong relationships with clinicians and stakeholders, identify opportunities for improvement and provide clinical leadership, education and support to enhance healthcare integration and patient outcomes.You will,Provide a key contact point for General Practice to navigate Metro South outpatient services and referral processes.Strengthen communication and coordination between primary and secondary care to enhance the patient journey.Build and maintain relationships with primary care clinicians through consultation, liaison, practice visits and resource coordination.About you:You are an experienced Registered Nurse with advanced clinical knowledge, strong problem-solving capability and the ability to build effective relationships across healthcare settings. You will bring a collaborative approach, confident communication skills and an understanding of General Practice, specialist outpatient services and referral pathways.
