Job summary

UnitingCare Health Pty Ltd. in Bowen/Proserpine seeks a Clinical Care Partner to oversee and coordinate complex clinical and care needs in the community.

You will assess, plan and link services to achieve optimal outcomes for aged care clients, spending much time on the road and collaborating with multidisciplinary teams. Essentially you will develop care plans, manage budgets, monitor client needs and assist with navigating the care system.

Qualifications

• Registered Nurse or Allied Health professional with current AHPRA registration and unlimited work entitlements.
• Demonstrated understanding of client-centred, community-based care.
• Experience with Community Funding streams and programs.
• Excellent communication and clinical problem-solving skills.
• Current Australian Driver’s Licence – Essential.
• Vehicle must be roadworthy and insured for work – Essential.
• Ability to work autonomously and manage time effectively.

Responsibilities

• Develop and review complex care plans with clients and the care team.
• Manage budgets and coordinate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs of community clients.
• Continuously monitor, review and evaluate care plans, services and outcomes.
• Assist clients in navigating the care system and linking to resources.
