Job summary

BlueCare in Caboolture & Surrounding Areas is seeking a Clinical Care Partner to oversee and coordinate complex clinical and care needs for community clients. You will assess, plan, and coordinate services to ensure person-centred, high-quality care and positive outcomes.

As a valued team member, you will spend most days on the road engaging with clients, supported by a large UnitingCare network and Not for Profit salary packaging options.

Qualifications

  • Registered Nurse or Allied Health professional with current AHPRA registration.
  • Current Australian Driver’s Licence and valid vehicle to use for work.
  • Ability to coordinate complex care plans and community-based services.

Responsibilities

  • Develop and review complex care plans with clients and the care team.
  • Manage budgets and coordinate multidisciplinary care activities.
  • Liaise with clinical teams to monitor changing needs of complex community clients.
  • Monitor, review and evaluate care plans, services and outcomes.
  • Assist clients in navigating the care system and connecting to resources.
  • Spend majority of day in the community visiting clients.

Job Description:

As aClinical Care Partneryour role is to ensure that our clients receive person-centred, high-quality care by overseeing andcoordinating complex clinical and care needs through assessment, care planning and coordination of services. Aiming to achieve the best care outcomes foreach client from their Aged Care Service funding through activities that include:

Develop and review complex care plans in collaboration with the client and multidisciplinary care team

Manage budgets and facilitate multidisciplinary care activities

Liaise with clinical teams to monitor and respond to changing needs of complex community clients

Continuously monitor, review and evaluate care plans, services and outcomes

Assist clients in navigating the care system and linking them to necessary resources where required

With no two days ever the same. You’ll spend the majority of your day on the road, engaging with clients within the community.

Role requirements:

Registered Nurse or Allied Health professionalholding a current AHPRA registration and unlimited work entitlements Essential

Current Australian Driver’s Licence - Essential

Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential

Demonstrated understanding and sound knowledge of client centred, community-based care services delivery

Demonstrated experience and knowledge in Community Funding streams and programs

Excellent communication and clinical problem-solving skills

Confidence in managing your time effectively, including an ability to work autonomously.
