Job summary

For Purpose Aged Care is seeking a Clinical Care Coordinator for our new Wallan Community Aged Care home. You will provide daily clinical leadership to RN/EN/PCA teams, working with Clinical Managers to implement quality care systems and ensure exceptional resident experiences.

We value hands-on leadership, compassionate culture, and strong oversight of clinical planning, staffing, and commissioning activities as we establish for this new site.

Qualifications

• Registered Nurse with current AHPRA registration.
• Strong clinical knowledge and assessment capability.
• Experience in staff leadership, supervision and development.
• Excellent communication and interpersonal skills.
• Understanding of funding, accreditation and compliance requirements.

Responsibilities

• Provide daily clinical leadership to RN/EN/PCA teams.
• Collaborate with Clinical Managers to embed high-quality care systems.
• Lead mentoring and clinical system implementation for the new home.
• Drive clinical outcomes while fostering compassionate care culture.

About The Role

As our Clinical Care Coordinator (CCC), you will provide daily clinical leadership and guidance to your team of RN’s, EN’s and PCA’s. Working closely with the Clinical Managers, you will help shape and embed highquality clinical care systems and support the delivery of exceptional resident experiences. Your role will be pivotal in providing handson clinical leadership and mentoring, driving strong clinical outcomes, fostering a compassionate and professional care culture, and supporting commissioning activities as clinical systems are established for a brandnew home.

What You'll Bring

• Minimum 2+ years’ experience in a Clinical Care Coordinator or similar leadership role within aged care or acute care
• Registered Nurse with current AHPRA registration
• Strong and uptodate clinical knowledge and assessment capability
• Demonstrated experience in staff leadership, supervision and professional development
• Excellent communication, interpersonal and negotiation skills
• Sound understanding of funding, accreditation and compliance requirements
• Experience with care planning and residentcentered clinical systems
