ATSICHS Aboriginal and Torres Strait Islander Community Health Service Brisbane – Brisbane QLD is seeking a qualified nurse for a contract role. The position requires Aboriginal or Torres Strait Islander status as a genuine occupational requirement.

Starting salary of $85,214 plus 12% super is offered, with salary packaging and fringe benefits. You will coordinate chronic disease care plans, health checks, and community engagement while upholding culturally safe practices in a not-for-profit

Duties

• Provide care coordination to assist in the provision of chronic disease management.
• Ensure appropriate Management and Coordination of individual Chronic Disease Care Plans, Cycle of care and Health Checks are up-to‑date.
• Promote and assist clients with complex care to a model of self-care.
• Review clients with collaborative case conferencing.
• Establish and maintain relationships with key external stakeholders conducive to holistic health and social support needs.
• Ensure processes are in place for regular feedback from client making necessary corrective actions.
• Engage with the community in a culturally sensitive manner, ensuring respectful and effective interactions.
• Foster a culture of continuous improvement and innovation within the Primary Health Support Services, integrating culturally safe practices.
• Ensure effective implementation and maintenance of ATSICHS Policy and procedures in line with AGPAL Accreditation Standards.
• Operational quality improvement and the use of PDSA cycles to improve everyday operations at a local level as presented in regular staff meetings.

Required

• Minimum Diploma in Nursing.
• Full un-restricted Enrolled Nurse AHPRA registration.
• Previous demonstrable experience in relevant nursing field.
• Demonstrated ability to meet targets and performance outcomes.
• Ability to support a continuous improvement model for achieving outcomes.
• Decisive listening, influencing and persuasive skills.
• National Police Certificate.
• Immunisations Category A.
• Driver's Licence.
• AHPRA Registration.
• Evidence of Work Rights.

Preferred

• Minimum two (2) years' experience in a primary health setting.
• Previous experience of working with Aboriginal and Torres Strait Islander clients.
• Previous experience in a multidisciplinary health team.
• Previous experience in chronic disease management.
