Job summary

Dementia and Aged Care Services is seeking an experienced Client Relations Manager to oversee a portfolio of clients receiving Support at Home services on the Central Coast. You will build trusted relationships with clients and families, coordinate quality services and manage budgets to ensure funding is used effectively.

Qualifications

• Experience in Support at Home, Home Care Packages or community aged care.
• Background in care management or care coordination.
• Ability to manage client budgets and funding utilisation.

Responsibilities

• Manage a portfolio of ~50–60 Support at Home clients.
• Develop and review goal-focused care and service plans.
• Coordinate services that promote wellness, independence, reablement and social connection.
• Manage client budgets, expenditure and service utilisation.
• Help clients understand funding, fees and service options.
• Build strong relationships with clients, families and service providers.
• Monitor service quality and respond to changing needs, risks or concerns.
• Maintain accurate records and meet quality, compliance and reporting requirements.
• Strengthen local referral networks and support service growth.
