Qualifications

• AHPRA registered Enrolled or Registered Nurse.
• Experience in chronic condition management preferred.
• Strong communication and organisational skills.

Responsibilities

• Prepare Care Plans and Team Care Arrangements.
• Support patients in managing chronic conditions and improving health behaviours.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams.
• Handle CCM appointment administration.
