Job summary

ForHealth Group in Marion, South Australia, is seeking a Registered or Enrolled Nurse to support GPs and patients through chronic condition management and preventative care. You’ll be part of a supportive team at Marion Domain Medical Centre, focused on quality outcomes and patient-centred care.

You’ll conduct health assessments, promote preventive initiatives, collaborate with multidisciplinary teams, and assist with CCM appointment administration.

Qualifications

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support patients in managing chronic conditions with care planning and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.
