Job summary

CatholicCare Sydney’s SPARK program seeks a Care Coordinator for Children and Families in Sydney’s Inner West. You’ll provide early intervention therapeutic support to children aged 5–11 and their families within a multidisciplinary team, across centre, home, school and community settings.

You will conduct assessments, coordinate family-centred support plans and help families navigate health, education and community services, while documenting outcomes and participating in case discussions for

Qualifications

• Tertiary qualification in Social Work, Psychology, Counselling, Developmental Education, Community Services or a related discipline.
• Experience in care coordination, case management, family support or child and family services.
• Sound knowledge of child development, attachment theory, trauma-informed practice and child mental health.
• Experience facilitating individual and group-based therapeutic programs.

Responsibilities

• Build strong and trusting relationships with children and families.
• Conduct assessments and identify child and family strengths, needs and goals.
• Develop and coordinate family-centred support plans.
• Support families to navigate health, education and community service systems.
• Facilitate referrals and connections to appropriate services and supports.
• Collaborate with schools, health professionals and community organisations.
• Advocate for equitable access to support.
• Maintain accurate documentation and outcome reporting.
