Qualifications

• Demonstrated experience in aged care, rehabilitation, or palliative care settings.
• Competence in functional and cognitive assessment tools relevant to ACRPC populations.
• Ability to work independently with confidence while contributing to a multidisciplinary team.
• Familiarity with SA Health clinical documentation systems or willingness to learn quickly.
• Understanding of NALHN's commitment to culturally safe, patient-centred care.

Responsibilities

• Conduct functional assessments and develop tailored intervention plans for patients across ACRPC streams
• Collaborate with multidisciplinary teams to support safe, person-centred discharge planning
• Prescribe and trial assistive equipment and home modification recommendations
• Contribute to clinical handover, documentation, and quality improvement within NALHN standards
