Job summary

SDN Children's Services is seeking a Case Worker for the Families Together program in the Northern Sydney District. You will support families experiencing complex challenges through regular home visits, delivering intensive, child-centred support tailored to each family’s needs within a collaborative team.

You will work in a hybrid model, using culturally responsive and trauma-informed practice, completing assessments, developing family-led plans, coordinating supports, and strengthening

Qualifications

• Relevant degree in social work or equivalent.
• Experience in community services or child and family sector preferred.
• Knowledge of NSW child protection legislation and risk management.
• Ability to work with diverse families and cultures.

Responsibilities

• Manage a caseload of 7-9 families.

About The Role:
As a Case Worker in the Families Together program, you'll support families experiencing complex challenges across the Northern Sydney District. Through regular home visits, you'll build trusted relationships and deliver intensive, child-centred, strengths-based support tailored to each family's needs. Using culturally responsive and trauma-informed practice, you'll complete assessments, develop family-led plans, and coordinate practical and therapeutic supports. Working alongside families and partner services, you'll help strengthen parenting capacity, reduce risk, and improve safety and wellbeing so children and young people can remain safely with their families. Duties of the role will include:

• Managing a caseload of 7-9 families
• Engaging families through home visits, assessment, and planning, including assessment of strengths, needs and risk and safety planning
• Developing and implementing project plans, coordinating referrals and supports to meet identified needs
• Providing practical support, coaching, and guidance to strengthen parenting capacity, communication, and problem-solving skills
• Delivering services in line with organisational frameworks, policies, procedures and child wellbeing outcomes
• Maintain accurate and timely documentation
• Work in partnership with children, young people and families, internal and external stakeholders.

About You:
You will have experience and skills in the following:

• University qualifications in social work or equivalent
• Experience in the community services or child and family sector is highly regarded
• Strong commitment to child-focused practice with a thorough understanding of child protection, legislation, and risk management
• Experience working with vulnerable and diverse children and families, including those from culturally diverse backgrounds
• Empathetic, supportive, and able to make sound judgments to protect children's rights and wellbeing
• Willingness to learn and apply feedback to improve clinical practice
• Current NSW Driver’s Licence
