Job summary

2b Inclusive is seeking an experienced Social Worker to join our Pathways to Success Program, supporting people with disability who experience complex mental health and psychosocial needs. This hands-on role focuses on case management, relationship-building and helping participants navigate often challenging service systems.

You will work with participants to identify barriers, connect them with the right services, advocate when needed and follow through beyond referrals, with travel across

Qualifications

• Bachelor or Master of Social Work is required.
• Experience as a social worker in health, disability or mental health sectors preferred.
• Strong communication, advocacy and service-navigation skills.
• Willing to travel regionally in South Australia.

Responsibilities

• Provide intensive, hands-on, person-centred case management.
• Support people experiencing complex mental health, psychosocial and disability needs.
• Complete assessments and develop practical case management goals.
• Help participants access mental health, health, housing and community services.
• Advocate with participants to overcome barriers.
• Coordinate with GPs, mental health teams, government agencies and other professionals.
• Provide assertive engagement and outreach for hard-to-engage individuals.
• Attend case conferences and multidisciplinary meetings.
• Maintain clear, timely case notes and documentation.
• Travel within metropolitan Adelaide and regional SA; regular day trips.
