Job summary

CatholicCare Tasmania in Burnie and Devonport seeks a caring Case Manager for the Care Finder Program on a part‑time basis. You’ll help vulnerable older people access aged care and community services through outreach, referrals and coordinated support.

Working in the North West, you’ll build partnerships with community organisations, maintain accurate client records and contribute to reporting on program outcomes. A supportive, values‑driven team environment awaits.

Qualifications

  • Experience with older people, My Aged Care, and/or the community services sector.
  • Well-developed case management, client engagement or support coordination skills.
  • Ability to liaise with community organisations and build networks to promote service.
  • Strong administrative skills, including maintaining client records and reports.
  • Excellent communication skills and the ability to engage with people from diverse backgrounds.
  • Proactive approach, sound judgement and ability to work autonomously.
  • Strong time management and ability to contribute within a small team.
  • Experience implementing and evaluating services with vulnerable people and families.

Case Manager (Care Finder Program)
Based in Burnie or Devonport
Part Time

CatholicCare Tasmania (CCT) is the primary social services agency of the Catholic Church across Tasmania. We have 60 years’ experience delivering a wide range of services and support to improve individual, family and community wellbeing. Our services are available and open to all with a particular focus on families, young people and children.

As part of our Care Finder team, you'll work directly with vulnerable older people who require intensive support to navigate and connect with required services. Through outreach, advocacy, service coordination and practical assistance, you'll help connect people with My Aged Care, health, community and other support services that improve wellbeing and quality of life. Care Finder Program is supported by funding from Primary Health Tasmania through the Australian Government’s Primary Health Networks Program.

Based in Burnie and/or Devonport, this role services Tasmania's North West region and offers the opportunity to make a genuine difference in the lives of some of our most vulnerable community members.

What you'll be doing

About you

You are passionate about supporting older people and have the initiative, empathy and organisational skills needed to work with individuals experiencing vulnerability and complex circumstances.

To be successful, you will have:

  • Experience working with older people, My Aged Care, and/or the community services sector.
  • Well-developed case management, client engagement or support coordination skills.
  • The ability to build effective partnerships and networks within the community.
  • Strong administrative skills, including maintaining client records and reports.

Selection Criteria
  1. Experience with working with older communities, My Aged Care and community sector experience
  2. Ability to liaise with community organisations and build networks to promote service
  3. Maintain accurate client records, case notes and referrals and actively contribute to reporting requirements
  4. Clear verbal and written communication skills and the ability to work collaboratively with team members, stakeholders and external partners
  5. Demonstrated experience in exercising considerable initiative, discretion and professional judgement and accept personal responsibility
  6. Highly developed organisational and time management skills together with an ability to work as part of a small team
  7. Demonstrated skills and abilities, implementing and evaluating services with vulnerable people and families
