CatholicCare Tasmania in Burnie and Devonport seeks a caring Case Manager for the Care Finder Program on a part‑time basis. You’ll help vulnerable older people access aged care and community services through outreach, referrals and coordinated support.

Our services are available and open to all with a particular focus on families, young people and children.
As part of our Care Finder team, you'll work directly with vulnerable older people who require intensive support to navigate and connect with required services. Through outreach, advocacy, service coordination and practical assistance, you'll help connect people with My Aged Care, health, community and other support services that improve wellbeing and quality of life. Care Finder Program is supported by funding from Primary Health Tasmania through the Australian Government’s Primary Health Networks Program.
Based in Burnie and/or Devonport, this role services Tasmania's North West region and offers the opportunity to make a genuine difference in the lives of some of our most vulnerable community members.
