Central Highlands Rural Health in Sunbury, VIC is seeking a Care Partner to join the Care at Home team. You will coordinate services for older people at home, complete assessments, build relationships with clients and families, and support clients to stay independent.

This is a permanent, part-time role with hours to be negotiated; salary is $48.87 per hour plus super.

CHRH is seeking a passionate and motivated Care Partnerto join our growing Care at Home team. In this rewarding role, you will work closely with older people and their families to coordinate services, support goal achievement, and deliver a positive, person-centred care experience that enables clients to remain living safely and independently at home.
