This isn’t your average case management role — with the rollout of the Support at Home Program, you’ll have the chance to lead with impact, empower clients, and help older Australians live life their way.

And yes… you’ll get a company car to travel around Bundall, Tweed Heads, and the beautiful surrounding suburbs.

Overview

As a Care Partner, you’ll be the go-to person for coordinating care that truly puts people first. You’ll work side by side with clients and families to navigate services like allied health, nursing, and home support.

Think of yourself as the connector, advocate, and coach — helping clients stay independent while living life on their own terms.

What You’ll Be Doing
  • Create and deliver personalised care plans

About You
