The Role

Reporting to the Operations Manager, you’ll play a pivotal role by delivering exceptional person-centred caremanagement to our elderly clients in their own home. You’ll be more than just a casemanager - you’ll be an advocate, problem-solver, and partner in helping our elderly clients live fulfilling, independent lives.

What You’ll Be Doing

• Work closely with home care package recipients to design care plans thatprioritise independence, well-being, and dignity.
• Collaborate with our internal teams to ensure the seamless delivery of high-quality care.
• Connect with clients, families, and stakeholders to foster trust, engagement, andexceptional outcomes.
• Identify risk factors, engage in a co-management approach to care planningensuring appropriate referral of clients to specialised services.
• Ensure integrity of client related documentation and data entry, maintainingaccurate and timely records in Sai’s client care managementsystems

Who You Are

• You have a background in case management and aged care and are dedicated to making a difference.
• Bonus if you have demonstrated aged care experience as an Enrolled/Registered Nurse
• Thrive in a fast-paced, agile environment and love problem-solving and takinginitiative.
• Your ability to engage, advocate, and inspire shines through in everything you do.
