Job summary

Right at Home invites applications for a Care Manager to lead nursing and allied health coordination within our home care network. The role focuses on training, scheduling, and ensuring high-quality, client-centred care across all clients.

You will oversee the care coordination team, engage with hospitals and clinics, and drive continuous improvement while aligning with aged care standards and organisational policies. This is a full-time, on-site leadership position.

Qualifications

• Registered nurse, enrolled nurse, physiotherapist or occupational therapist with current AHPRA registration.
• Minimum of three years in a similar care management role.
• Knowledge of Australian aged care quality standards and legislation.
• Ability to lead and develop a care team and coordinate services.
• Strong communication and documentation skills.

Responsibilities

• Oversee training, development, and compliance of carers.
• Schedule and monitor client care, ensuring adherence to care plans.
• Build relationships with hospitals, clinics, and care facilities.
• Lead care coordination and nursing/therapist activities as required.
• Maintain clinical governance and quality improvement initiatives.
