A community health organization in Melbourne is seeking multiple Care Managers to join their supportive multidisciplinary teams. Ideal candidates will have at least 3 years of experience in Home Care Packages, with strong knowledge of consumer directed care. The role involves delivering quality case management services and developing care plans. This full-time position offers hybrid working arrangements and a competitive salary.
