Job summary

Yackandandah Health, part of the Apollo Care Alliance, is seeking a compassionate Care Manager to lead our 83-bed home and support older Australians to live with dignity. You will guide the care team, ensure safe, high-quality, resident-centred care, and oversee planning, governance, and compliance with aged care standards.

Reporting to the Residential Service Manager, you will promote resident choice, family engagement, and continuous quality improvement.

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Minimum 5 years’ experience as a Clinical Nurse, Care Coordinator, or Care Manager (with at least 2 years in aged care).
• Strong leadership and team management skills.
• Excellent knowledge of Strengthened Aged Care Quality Standards and clinical governance.

• Lead and support the care team to deliver holistic, person-centred care.
• Oversee care planning, clinical governance, and resident assessments.
• Ensure compliance with regulatory requirements, including the Aged Care Act 1997.

About You:
