Job summary

Yackandandah Health, part of the Apollo Care Alliance, seeks a compassionate Care Manager to lead our care team in an 83-bed home in regional Victoria. Reporting to the Residential Service Manager, you will guide care delivery to ensure safety, quality, and resident-centred practice.

You will oversee care planning, governance, and assessments, promote resident choice and family engagement, and drive ongoing quality improvement in line with the Strengthened Aged Care Quality Standards.

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Minimum 5 years’ experience as a Clinical Nurse, Care Coordinator, or Care Manager (with at least 2 years in aged care).
• Strong leadership and team management skills.
• Excellent knowledge of the Strengthened Aged Care Quality Standards and clinical governance.
• Commitment to supporting residents to live with dignity and grace.
• Strong verbal and written communication skills.
• Compassionate and empathetic by nature.

Are you a dedicated Care Manager with a passion for leading care teams and supporting older Australians to live with dignity and grace? Yackandandah Health, part of the Apollo Care Alliance, is seeking a compassionate and experienced Care Manager to join our team.

About the Role

As a Care Manager, you will play a key leadership role in ensuring the delivery of safe, high-quality, resident-centred care in our lovely 83 bed home. Reporting to the Residential Service Manager, you will guide and support our care team, ensuring compliance with the Aged Care Quality Standards and Apollo Care’s model of care.
