Job summary

DPV Health is seeking a Care Finder to support people in understanding and accessing aged care services within Melbourne's northern region. The role focuses on assertive outreach, needs assessment, and coordinating with community supports to connect clients with appropriate services.

Candidates will have a tertiary qualification in Human Services or Social Work, strong analytical skills, and experience with diverse communities. A consumer‑focused, collaborative approach is essential.

Qualifications

• Tertiary qualification in Human Services or Social Work.
• Experience in needs assessment, care planning and/or case management.
• Extensive knowledge of the aged care, health and community services system.
• Experience with older people and families from diverse backgrounds.
• Ability to work collaboratively and autonomously.
• Experience in community networking with external service providers.
• Computer literacy and the ability to manage a client database.
• Strong problem solving and customer service skills.
• Analytical skills.

Responsibilities

JOB DESCRIPTION

The Care Finder program was announced in the 2021-22 Commonwealth Budget as part of the Connecting Senior Australians to Aged Care Services measure. Care Finders will:

• Provide specialist and intensive assistance help people to understand and access aged care and connect with other relevant supports in the community; and
• Specifically target people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the Mitchell and Whittlesea shire
• Provide assertive outreach while building engagement and rapport with potential clients
• Guide clients through the assessment process and assist in obtaining Aged Care supports and services needed

About the Candidate

• Tertiary qualification in Human Services/Social Work or related profession and or previous experience in needs assessment, care planning and/or case management
• Extensive knowledge of the aged care, health and community services system and regulatory environment
• Experience in working with older people and their families/carers from a range of backgrounds, and an understanding of the barriers they may experience in accessing aged care services
• The ability to work collaboratively and autonomously and build effective relationships
• Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client focused services.
• Computer literacy and the ability to effectively manage a client data base
• High level problem solving, exceptional customer service skills and analytical skills
