Job summary

NHS Jobs in Australia seeks a PCN coordinator to support primary care network priorities and drive engagement with practices and communities. The role involves coordinating outreach, audits, and liaising with PCN leads to deliver health promotion and vaccination programmes.

You will collect feedback, support decision-making aids, and contribute to DES/LES specifications while maintaining strong engagement across practice staff and partners.

Responsibilities

• Undertake work in line with PCN directed priorities.
• Manage PCN Social Media
• Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids
• Support Link worker manager, with appointment audits and coordinator of outreach work
• Support PCN Neighbourhood leads
• Support PCN Clinical director and PCN manager
• Send Patient surveys for PCN Practices
• Raise awareness of health promotion in PCN Practices
• Support PCN Leads with uptake of national screening programmes
• Support PCN Leads with immunisation programmes
• Support delivery of the PCN covid vaccination programme
• Direct liaison with multi agencies to coordinate care for patients
• Lead PCN patient participation group
• Run PCN Extended access audits
• To support patient/carer contact roles, and collate patient and carer feedback on their experiences
• Support Quality and Outcome Frameworks and other DES/LES specifications
• Maintain and develop PCN engagement with all practice staff and encourage best practice
• Perform any other duties assigned by management
