Job summary

Ability Action Australia in New South Wales is seeking a Behaviour Support Practitioner to support the Gunnedah region. The role is open to all experience levels and offers ownership of your caseload in a supportive environment.

You'll assess and plan with NDIS participants and families, deliver evidence-based support, and collaborate within a multidisciplinary team. Flexible hours available; part-time or full-time with work-from-home options.

Qualifications

• A degree qualification as an allied health professional.
• Experience in report writing.
• Strong organisational skills and the ability to manage a varied caseload.
• A proactive, self-motivated approach, with a drive to improve outcomes.

Responsibilities

• Supporting NDIS participants and families to identify needs through assessment and person-centred planning.
• Owning the assessment, development and delivery of evidence-based support plans within a complex environment.
• Driving meaningful goals and outcomes using a positive behaviour support framework and reducing restrictive practices.
• Ensuring plans reflect each participant's goals, strengths and what matters most to them.
• Building capability, confidence and understanding of those supporting participants in daily life.
• Collaborating within a multidisciplinary team.
