Job summary

IDHS invites applications for the Care Partner / Case Manager – Support at Home in Mildura, Victoria. The role coordinates and oversees a client-centred support at home program for seniors, acting as main liaison for clients and families to ensure timely, appropriate services and outcomes.

Responsibilities include care planning, service coordination, risk management and advocacy, with a focus on consumer choice, safeguarding, and culturally appropriate care within the Australian aged care system.

Qualifications

• Diploma or higher qualification in Community Services, Aged Care, Nursing, Health or Human Services; relevant experience considered.
• Minimum 2 years in Support at Home or Home Care Package programs.
• Experience coordinating complex or higher-risk clients and working with cognitive decline, social isolation, or multiple needs.

Responsibilities

• Coordinate and oversee a high-quality, person-centred Support at Home service.
• Act as the key contact for clients, families and representatives; align services with individual goals.
• Develop and review care plans; monitor wellbeing and outcomes; identify risks and advocate for timely support.
• Monitor budgets, document accurately, ensure legislative and organisational compliance.
