DFP Recruitment is seeking experienced and empathetic professionals to deliver high-quality, person-centred support needs assessments for people living with disability in Queensland.

You will gather and analyse information, interpret evidence, and prepare clear assessment reports to support informed decisions, while communicating respectfully with participants, families and support networks in trauma-informed and culturally-safe ways.

Qualifications

  • Experience conducting support needs assessments or similar person-centred assessments.
  • Strong analytical, report writing and decision-making capability.

  • Trauma-informed and strengths-based approaches to participant engagement.

Responsibilities

  • Conduct support needs assessments through guided conversations and information gathering.
  • Analyse and synthesise participant information and external evidence to support evidence-based outcomes.
  • Prepare clear, accurate and defensible written assessment reports.
  • Apply policy, legislation and procedures using sound judgement and ethical practice.
  • Communicate assessment processes and outcomes clearly across virtual and face-to-face settings.
  • Escalate complex or high-risk matters appropriately and contribute to continuous service improvement.
  • Collaborate across teams to support coordinated and inclusive participant outcomes.
  • Maintain accurate records using digital systems while ensuring privacy and confidentiality.

Job description

We’re seeking experienced and empathetic professionals to deliver high-quality, person-centred support needs assessments for people living with disability.

In this role, you will gather and analyse information, interpret evidence, and prepare clear, accurate assessment reports to support informed decision-making. You’ll communicate respectfully with participants, families and support networks, using trauma-informed and culturally-safe approaches to ensure people feel heard, understood and supported.

Key Responsibilities will include:
  • Conduct support needs assessments through guided conversations and information gathering.

  • Prepare clear, accurate and defensible written assessment reports.
