Hunter Valley Transitional Aged Care Program (TACP) is multi-disciplinary team supporting and providing aged care clients with the opportunity to improve their health and wellbeing after discharge from hospital. The TACP team comprises of Allied Health and Nursing professionals working closely with the clients and their families using an enablement focus, providing slow stream therapy aiming to improve the clients function, independence and safety at home. The service operates as a home visiting service, dependent upon patient needs. The service operates 8:00am - 4:30pm Monday - Friday all year round.
The Allied Health Assistant (AHA) supports clients’ needs under the supervision of Allied Health professionals, including Occupational Therapists, Physiotherapists, Dietitians, Speech Pathologists and Social Workers. The role may include assisting clients with prescribed Allied Health programs, exercise classes, social groups, equipment needs, nutritional requirements and transportation, as required. The position reports to the Nursing Unit Manager, with clinical supervision provided by an Allied Health professional. A comprehensive orientation and all required mandatory education will be provided. Ongoing clinical supervision will support the AHA to develop the knowledge, skills and confidence required to learn and master all aspects of the role. A fleet vehicle is provided for all work-related travel, and there are ongoing opportunities for professional development and education. Clinical supervision will enable all facets of the position to be learned and mastered.
If you are punctual, reliable, empathetic and client focussed, look no further. Experience in the role would be ideal but not mandatory. Computer literacy would be beneficial and the ability to work in a team share knowledge and prioritise task is required.
