Job summary

BlueCare is seeking a Wellness Partner for Flinders View Nowlanvil Aged Care Facility. You will deliver engaging, person-centred wellness programs that support residents’ physical, emotional, social and cognitive wellbeing.

Work closely with residents, families and the care team to create meaningful participation, independence and connection. You will facilitate activities, build relationships, support independence and collaborate with a multidisciplinary team to deliver quality care and

Qualifications

• Certificate IV in Leisure and Health or equivalent qualification.
• Current Driver’s License - willing to drive 12-seater bus.
• Experience delivering group and 1:1 activities in aged care or community settings.
• Practical experience in creative, social, cognitive and physical activity facilitation.
• Understanding of dementia care, person-centred practice and cultural safety.
• Knowledge of infection control, manual handling and Aged Care Quality Standards.
• Exposure to person-centred care principles and needs of older adults.
• Experience in multidisciplinary teams including nurses, allied health and volunteers.
• Strong ability to build positive, respectful relationships with residents and families.
• Ability to design/adapt lifestyle and wellness activities for residents with cognitive or physical limitations.
• Understanding of dignity, cultural safety and individual choice in care delivery.
• Probity checks as required by legislation/policy.

Responsibilities

• Facilitate group and one-on-one wellness activities tailored to residents’ interests and abilities.
• Build positive relationships with residents through meaningful engagement and companionship.
• Support residents’ independence, choice, dignity and wellbeing.
• Collaborate with Wellness Lead, care teams, families and volunteers to deliver engaging programs.
• Assist with wellness assessments, documentation and reporting changes in resident wellbeing.
• Support volunteers to contribute confidently to resident activities.
• Prepare and maintain safe, welcoming activity spaces and equipment.
• Follow organisational policies and contribute to continuous improvement initiatives.
