Job summary

Guide Healthcare is seeking a passionate Physiotherapist to join our Allied Health team. You will support clients in aged care, home care, and disability settings, focusing on strength, mobility and confidence to live independently. Role blends community-style care with on-site teamwork.

You will deliver assessments, care planning, manual handling training, and wellness programs, while maintaining flexibility to meet residents' goals and needs within HELF frameworks.

Qualifications

• AHPRA registration is required.
• NDIS Worker Check is required.
• National Police Check is required.
• Driver’s licence and reliable car necessary for Home Care if want more than 2 days/wk.

Responsibilities

• Assess, plan and deliver evidence-based, goal orientated, and outcome-driven therapy.
• Build a vibrant wellness culture that enhances resident outcomes and engagement.
• Deliver Wellness and Reablement Programs within HELF services.
• Prescribe and document exercise programs for residents; collaborate with onsite S&C team.
• Build strong client relationships and deliver therapy 1:1s and group programs.
• Document clearly and efficiently; report against HELF service KPIs.
