Job summary

Opal HealthCare invites applications for a Care Manager role at Rutherford Park Care Community, Hunter region. You will lead the clinical and care workforce, delivering safe, high-quality person-centred care aligned to the Aged Care Act 2024 and Quality Standards. This hands-on leadership role balances governance, capability, and performance.

Reporting to the General Manager, you will partner with regional teams to drive clinical excellence and high-performing teams across the site.

Qualifications

Registered Nurse with AHPRA registration and at least 3 years of experience.
Experience leading clinical teams in residential aged care.
Strong understanding of Aged Care Quality Standards and AN-ACC.
Ability to lead change, manage risk and improve outcomes.
Values-led leadership with mentoring capabilities.

Responsibilities

Lead clinical care delivery ensuring compliance with Aged Care Quality Standards and regulatory obligations
Oversee care planning, admissions reviews, and changes in resident condition
Monitor and analyse clinical indicators, incidents and risk trends
Apply evidence-based practice to improve outcomes
Build relationships with GPs, hospitals and allied health providers
Ensure adherence to Aged Care Act 2024 requirements and reporting obligations
Drive continuous improvement, audit readiness, and clinical excellence
Lead compliance with incident management and accreditation standards
Foster a values-led, high-performing culture
Oversee education, onboarding and competency development
