Job summary

Portland District Health in Portland, Victoria, is seeking a suitably qualified candidate to join the Community Aged Care Services team on a fixed term part-time basis.

You will provide guidance, supervision and support for Health Care Workers delivering services under the Commonwealth Home Support Program and other funded community programs, helping older people and their carers to remain independent at home.

Qualifications

• Certificate III in Business Administration is required.
• Demonstrated ability to work collaboratively in a multidisciplinary health care team.
• Knowledge and experience delivering duties to support clients in the community.
• Excellent communication, interpersonal, written and ICT skills.
• Understanding of person-centred care aligned with PDH frameworks and policies.
• Understanding diverse backgrounds and cultures.
• Excellent communication and ability to provide friendly, courteous service.
• Desirable: experience in hospital, community care, or residential aged care.
• Desirable: Certificate III or IV in Aged Care.

Responsibilities

• Provide guidance and supervision for Health Care Workers in CHSP.
• Support clients to remain in their own homes through household tasks.
• Assist in delivering high-quality community aged care services.
