Job summary

Bayside Health in Frankston, Victoria, invites applications for a permanent part-time role supporting Advance Care Planning (ACP) and Voluntary Assisted Dying (VAD) Liaison Officer across Community Care and hospital settings.

The Advanced Care Planning Clinician and VAD Liaison Officer work with a team delivering ACP support, education and coordination with statewide navigators, ensuring patient-centred care. Allied health or nursing background preferred.

Qualifications

• Allied Health Practitioner with APHRA registration or eligible for registration.
• Solid understanding of Advance Care Planning aims and underlying principles.
• Experience in palliative care, end-of-life care, Advance Care Planning, or Voluntary Assisted Dying.
• Demonstrated commitment to person-centred care and patient advocacy.
• Workplace experience supporting and mentoring colleagues / staff.
• Willingness to learn, maintain and advance knowledge relevant to the position.
• Commitment to work as part of a specialised team to achieve the ACP Program aims.
• Ability to supervise undergraduate students.
• Current Victorian Driver's Licence.
• Aged Care Police Check.
• Working with Children Check.

Responsibilities

• Provide ACP support across Bayside Health and community settings.

Classification: Social Worker Grade 2 Year 1 - 4 (SC21 - SC24) and Registered Nurse Grade 3 (ZJ1) plus super

The Team & Role

Advance Care Planning (ACP) & Voluntary Assisted Dying Liaison Officer (VADLO) within Community Care, The Advance Care Planning and Voluntary Assisted Dying Liaison Officer will work across and support the both streams of the program.

The Advanced Care Planning Clinician works as part of a team who are responsible for providing ACP support across PH and in the community. The Voluntary Assisted Dying Liaison Officer (VADLO) works in collaboration with the VAD Statewide Care Navigators to provide a coordinated response to a patient's VAD request. The VADLO provides education and support to relevant clinical areas across the organisation.

Skills & Experience Required

• Allied Health Practitioner with current registration with Australian Health Practitioner Regulation Agency (APHRA) or eligible for registration with the relevant professional organisation (if Social Work).
• Sound understanding of Advance Care Planning aims and underlying principles.
• Experience in palliative care, end-of-life care, Advance Care Planning, and or Voluntary Assisted Dying
• Demonstrated commitment to person centred care and patient advocacy.
