Job summary

Cairns and Hinterland Hospital and Health Service is seeking an Aboriginal and Torres Strait Islander Health Worker - Advanced to deliver culturally safe care within Hospital in the Home and community settings in the Cairns region.

The role focuses on health promotion, prevention, and management of conditions, offering direct patient care at home and in inpatient settings, working closely with a Registered Nurse or CNC to maintain high standards of care.

Responsibilities

• Provide effective support and advocacy for HaITI clients by conducting timely home visits, case management conferencing, follow-up and referrals to relevant health services/programs.
• Coordinate and conduct ongoing health promotion and education sessions with individuals and families within the local Aboriginal and Torres Strait Islander community.
• Empowering consumers to make informed health care decisions through consultation, education and information sharing.
• Deliver evidence-based care by working collaboratively and in partnership with consumers, carers, external agencies and other health professionals.
