Job summary

Monash Health’s Adult Acute Social Work team at Dandenong Hospital is seeking a compassionate Grade 2 Social Worker for a full-time fixed-term role until November 2027. You will provide high-quality psychosocial assessments, discharge planning and bedside support to patients and families during challenging times.

You will collaborate with multidisciplinary teams, advocate for patient-centred care, and connect people with community supports.

Qualifications

• AASW-accredited Bachelor or Master of Social Work (eligibility for membership)
• Minimum two years post-graduate Social Work experience
• Strong psychosocial assessment and discharge planning skills
• Ability to work autonomously and within a multidisciplinary team

Responsibilities

• Undertake comprehensive psychosocial and risk assessments
• Support discharge planning and care coordination
• Provide counselling, advocacy and practical support to patients and families
• Work with multidisciplinary teams to ensure safe, patient-centred care
• Connect patients and families with appropriate community services
• Contribute to ongoing development of Social Work practice
